For your First Visit
In order for us to provide the best care for you we ask that you bring the following important items to your visit:
- Photo Identification
- Your insurance information and card
- A list of your current medications
- Your referring physician and primary care physician’s name, address, phone number, and fax number.
- Your referral/script from your physician.
- Your copay is due at the time of your visit.
